Hint 1: pls look on left side hand movement is restricted It's spastic gait with hemiparetic on left side upper limb more than lower limb Hint 2 : Short neck Hint 3 Left Upper limb muscles are wasted, weak and reflexes r brisk in all the 4 limbs Normal milestones no cranial neuropathy, no seizures Cranial hemiparetic lesion 7th nerve would also be involved Clinically it's high cervical cord compression It's basilar invagination with Atlanto axial dislocation


Moral of the Story

Another clinical dd would be Holocord Syrinx with chairi can also present in this way but there would be presence of dissociate anaesthesia Another clinical dd would cervical cord tumor
